These terms explain how Courtney Crichton Physiotherapy provides services, manages appointments and payments, and works with patients to support safe and effective care.
By booking or attending an appointment, you acknowledge that you have had the opportunity to read these terms and ask any questions. Your agreement to these terms does not replace the separate informed consent process required for your clinical care.
1. Services provided
Courtney Crichton Physiotherapy provides physiotherapy and related assessment, rehabilitation and exercise services within the practitioner's professional scope of practice.
Treatment recommendations are based on your presentation, assessment findings, goals and clinical needs. Outcomes vary between individuals, and no particular treatment result can be guaranteed.
You are encouraged to ask questions, discuss your preferences and raise any concerns about your care at any time.
2. Appointments and bookings
Appointments can be arranged through the clinic's available booking methods, subject to availability.
Please provide accurate contact details and relevant information needed to arrange your appointment. Notify the clinic if your contact details change.
Online bookings require a minimum $20.00 deposit at the time of booking and a larger fee for extended appointments. This is displayed transparently at the time of booking to secure the online booking spot. This deposit forms part of the total appointment fee and is deducted from the amount payable for the consultation.
The appointment time and service booked determine the applicable consultation fee.
3. Fees and payment
Current consultation fees will be communicated to you before or at the time of booking and are displayed on the website. The clinic will advise you of any relevant additional charges before proceeding with services that incur them.
Payment of the remaining appointment balance is due in accordance with the payment arrangements communicated by the clinic.
The clinic accepts the payment methods made available at the time of your appointment. Please contact the clinic if you need clarification about a fee or payment.
4. Booking deposits
A minimum $20.00 deposit is required for all online bookings and larger amounts for extended appointment times displayed at the time of booking.
The deposit is applied towards the total consultation fee and is not an additional fee.
Where an appointment is cancelled or rescheduled with at least 24 hours' notice, the deposit will be refunded or transferred to a replacement appointment, as agreed with the clinic.
For late cancellations and missed appointments, the clinic may apply a reasonable cancellation fee in accordance with its Cancellation and Appointment Policy. Any deposit paid will be credited towards that fee, and any excess will be refunded.
Where no cancellation fee is payable, the deposit will be refunded. Where the clinic cancels an appointment, the deposit will be refunded or transferred to another appointment at your choice.
Refunds will be processed using the original payment method where practicable. Processing times may depend on the payment provider.
5. Cancellations, rescheduling and missed appointments
Please provide at least 24 hours' notice when cancelling or rescheduling an appointment.
Late cancellations and non-attendance may result in a reasonable fee, taking into account the circumstances, the reserved appointment time and any reasonably unavoidable loss.
The clinic recognises that genuine emergencies and exceptional circumstances can arise. These will be considered individually, and discretion may be exercised where appropriate.

6. Late arrival
Please contact the clinic if you expect to arrive late.
The clinic will make reasonable efforts to accommodate late arrivals where practicable. However, your appointment may need to be shortened or rescheduled to avoid affecting other patients or compromising safe care.
The full appointment fee may remain payable where clinical time has been reserved for you.
7. Patient responsibilities
To help provide safe and appropriate care, you agree to:
Provide accurate and relevant information about your symptoms, medical history, medications, injuries and other factors that may affect treatment.
Tell your physiotherapist about changes in your health or symptoms.
Ask questions if you do not understand the proposed assessment, treatment, risks or costs.
Follow agreed home exercise or rehabilitation advice to the extent that it is appropriate and safe for you.
Let your physiotherapist know if an exercise or treatment causes unexpected pain, discomfort or other concerning symptoms.
Treat clinic staff and other patients respectfully.
Arrive on time for your appointment allocation
You may decline or withdraw consent to a proposed treatment. Your physiotherapist will discuss appropriate alternatives and any implications for your care.
8. Clinical assessment, informed consent and outcomes
Your physiotherapist will explain relevant assessment and treatment options, including material risks, expected benefits and reasonable alternatives, to support informed decision-making.
Your consent will be sought before assessment and treatment as required. You may ask questions or withdraw consent at any time.
Physiotherapy outcomes depend on many factors, including the nature of the condition, individual circumstances and participation in rehabilitation. The clinic does not guarantee a specific outcome.
These terms do not replace any additional consent forms, clinical discussions or instructions relevant to your individual care.
9. Children and patients requiring support
Parents, guardians, carers or support people may be involved where appropriate and authorised.
10. Private health insurance, Medicare and third-party payments
Eligibility for rebates, subsidies or third-party funding depends on the relevant scheme's requirements and your individual circumstances.
Patients are responsible for confirming their eligibility, referral requirements, limits and any gap payments that apply.
The clinic can provide appropriate receipts and documentation for services delivered. A rebate or payment from an insurer or other organisation cannot be guaranteed.
Any Medicare, workers compensation, motor accident or other funded service will be managed according to the requirements applicable to that service.
11. Privacy and confidentiality
The clinic collects and uses personal and health information for purposes such as providing treatment, maintaining clinical records, processing payments, managing appointments and meeting legal or professional obligations.
Information will be handled in accordance with applicable privacy and health records laws. It will not be disclosed to others without appropriate authority or consent, unless disclosure is permitted or required by law or is otherwise authorised.
You may contact the clinic to ask about access to your information or to request correction of inaccurate information, subject to applicable legal requirements.
12. Communication and appointment reminders
The clinic may contact you using the contact details you provide for appointment confirmations, reminders, scheduling changes, billing and other matters relating to your care.
Please keep your contact details up to date and advise the clinic if you have preferences about how it contacts you, subject to any communication needed for your care or legal obligations.
13. Feedback and complaints
If you have a concern about your care, fees, appointment arrangements or another aspect of the service, please contact Courtney Crichton Physiotherapy so the matter can be discussed and reviewed.
The clinic aims to respond respectfully and promptly and to work towards an appropriate resolution.
14. Changes to these terms
The clinic may update these terms from time to time. The current version will be made available to patients.
Any changes will apply prospectively, with reasonable notice where appropriate. Changes will not remove rights or remedies that are protected by law.
